Lower Back Pain Physio Perth

Perth's Evidence Based Lower Back Pain Treatment Experts

neck pain & back pain perth physio

There can be many causes to low back pain such as muscle overuse, strenuous activity, trauma, long-term injury, postural deficits, or even emotional stress can play an important role.

The team at Niche Physio and Pilates have a particular interest in lower back pain, in particular the diagnosis, treatment and management of spine-related disorders. We provide a targeted hands-on approach to treatment initially, which is followed by an individualised rehabilitation program focused on restoring your optimal function. All our physiotherapists have qualifications in clinical Pilates and have a strong focus on exercise rehabilitation. We work closely with a great team of local Sports Physicians, Pain Specialists and Radiology clinics around the area; enabling you to have the best possible outcome. We strive to maintain a holistic approach through patient self-management strategies, addressing mental health concerns, sleep disturbances, practicing poor habits such as smoking, or overall poor health such as being overweight.

When surgery is required, the team at Niche Physio and Pilates will guide you through both the pre and post-operative stages with the utmost care, working closely with your surgeon to ensure the best possible outcome is achieved. Sometimes it is a simple ‘quick fix’ for an easy muscle strain or restricted joint, but most times it can be more complicated, and we are here to help.

What lower back pain actually feels like

People describe it very differently depending on what is driving it. A deep ache across the belt line that builds after standing. A sharp catch bending to put shoes on. A spasm that seized on the way up from the garden and has not let go since. Pain down the back of one leg past the knee. Or a back that is simply stiff and sore every morning for the first twenty minutes.

Almost all of it is treatable, and the great majority of it is not caused by anything structurally sinister — even when it is severe.

What we treat, and what tends to cause it

Non-specific lower back pain

The most common presentation by a distance. There is no single damaged structure to point at; the pain comes from a combination of joint irritation, muscle guarding and load that has outstripped what the back is currently conditioned for. It is genuinely painful and it responds well to treatment and graded loading.

Disc-related pain

Often worse with sitting, bending and first thing in the morning, and eased by walking or standing. May or may not refer into the buttock or leg. Most disc presentations settle well without surgery.

Sciatica and nerve pain

Pain travelling below the knee, sometimes with numbness, pins and needles or weakness. It usually needs a longer runway than simple back pain, and it needs proper assessment — including a neurological examination — rather than a generic exercise sheet.

Facet joint irritation

Typically one-sided, worse with leaning backwards or twisting, and often eased by sitting or bending forwards. Responds particularly well to hands-on treatment combined with specific control work.

Postnatal and pelvic-related back pain

Back and pelvic pain after pregnancy has its own considerations, including the abdominal wall and pelvic floor. Our women’s health physiotherapists assess this specifically rather than treating it as ordinary back pain.

When to seek help sooner

Most back pain is safe to manage conservatively. Book an urgent appointment or see a doctor promptly if you have numbness around the groin or inner thighs, difficulty controlling your bladder or bowels, progressive weakness in a leg, unexplained weight loss or fever alongside the pain, or if it followed a significant fall or accident. These are uncommon, but they change the plan.

What happens in your first appointment

An Initial Physiotherapy Consult runs for 30 minutes. If you have a lot to cover, or two separate problems, book a Long Consult at 60 minutes.

  1. We listen first. How it started, what aggravates and eases it, how it affects sleep and work, and what you need to get back to.
  2. We assess. How your spine and hips move, which movements reproduce your symptoms, how the trunk and hip muscles are working, and a neurological screen if there is any leg involvement.
  3. We explain what we find — including what your pain most likely is, and what it almost certainly is not. For a lot of people this part does more than the treatment.
  4. We treat on the day, which may include hands-on joint and soft tissue work, dry needling, and taping.
  5. You leave with a plan — specific exercises, guidance on movement and load, and a realistic timeframe.

How long recovery usually takes

Acute lower back pain often improves markedly within two to six weeks. A first episode can settle faster than people expect once the initial spasm eases.

Pain that has been present for months behaves differently. It is still very treatable, but the work shifts from settling symptoms toward rebuilding tolerance, and that is measured across a couple of months rather than a couple of visits. Sciatica sits somewhere in between and is best judged after assessment.

Two things reliably slow recovery down: complete rest, and avoiding all the movements that once hurt long after they have stopped being a threat. Modified activity beats bed rest in essentially all current guidance.

How we treat it

We use a targeted, hands-on approach to settle symptoms, followed by a rehabilitation program aimed at restoring normal function and tolerance to load. All of our therapists are certified clinical Pilates instructors with a strong exercise rehabilitation focus, so moving from pain relief into structured strength work happens in the same clinic with the same people.

Where it fits the presentation we also use dry needling, and clinical Pilates in groups of three for people who want closely supervised rehab. If your back pain is tied to a specific goal — returning to running, lifting at work, getting through a season — say so at the first appointment and we will build the plan around it.

Common questions

Should I rest or keep moving?

Keep moving, within reason. Current guidelines are consistent on this: staying active and returning to normal activity as symptoms allow produces better outcomes than bed rest. That does not mean pushing through severe pain — it means avoiding the trap of complete rest, which tends to make backs stiffer and more sensitive.

Do I need an MRI or X-ray?

For most lower back pain, no. Scans of people with no pain at all routinely show disc bulges and degeneration, so imaging often finds things that are not the cause of your symptoms and can make people more fearful of moving. We image when the assessment suggests it will change the treatment plan, and we will explain why if we think you need it.

Is my disc slipped, and can it be put back?

Discs do not slip in and out of place — they are firmly attached. A disc can bulge or herniate and irritate nearby structures, and that can be genuinely painful, but nothing is being relocated by any treatment. The good news is that most disc-related pain settles well with conservative management.

How long before I can get back to the gym or running?

Often sooner than people assume, in a modified form. Rather than waiting until you are pain free, we usually find a version of the activity you can tolerate now and build from there. Bring your training schedule to the first appointment.

Do I need a referral?

No. You can book physiotherapy directly in Australia. A referral is only needed for particular funding arrangements such as a Medicare care plan, workers compensation or a motor vehicle claim.

What about private health, NDIS or workers compensation?

Physiotherapy is claimable if you hold extras cover that includes it — bring your card and we will sort out the claim with you. We also treat clients under NDIS, workers compensation, motor vehicle and DVA arrangements. If you are not sure which applies, call the clinic on (08) 9242 1102 before booking.
